Exertional substernal chest pressure relieved by rest or nitroglycerin, lasting minutes, reproducible with exertion/emotion.

Resting ECG first (often normal); then exercise ECG stress test. Use pharmacologic stress with imaging if the patient cannot exercise or the baseline ECG is abnormal (LBBB, paced, LVH with strain).

Antianginal: beta-blocker first-line (add CCB, long-acting nitrate, or ranolazine). Mortality/risk: aspirin + high-intensity statin + risk-factor modification; revascularize for refractory symptoms or high-risk anatomy.

Both have rest/crescendo chest pain and ECG showing ST depression or T-wave inversion. NSTEMI has elevated serial troponin; unstable angina has normal troponin.

Aspirin + P2Y12 inhibitor (dual antiplatelet), anticoagulate with heparin, plus nitrates/beta-blocker/statin/ACEi; risk-stratify with TIMI/GRACE. Early invasive angiography if high-risk.
